Job summary

Monarch Communities is seeking a Wellness Nurse (LPN) in New York. This per-diem, non-exempt role reports to the Health and Wellness Director and involves direct nursing care with supervision of care staff. Salary range is $38.00-$40.00 per hour with all shifts available.

The position requires an active LPN license, 2+ years in relevant settings, and ability to lift up to 50 lbs. Join a compassionate team focused on holistic resident wellness and compliance with regulations.

Qualifications

• Must hold a valid LPN license and current NY licensure.
• At least 2 years’ experience in long-term care, assisted living, home health or hospital setting preferred.
• Experience working with residents with Alzheimer’s or other dementias.
• Ability to handle multiple tasks and prioritize responsibilities.
• Knowledge of federal and state nursing regulations and best practices.
• Proficiency in computer skills (Microsoft Office) and ability to learn new systems.

Responsibilities

• Monitors the health, safety, and well-being of all residents.
• Assists in training and monitoring of medication administration.
• Supervise care staff in accordance with state regulations and community policies.
• Maintains clinical quality assurance in line with federal, state, and local standards.
• Promotes the highest degree of service to residents and upholds the company mission.
• Conducts thorough resident assessments.
