Job summary

Monarch Communities is seeking a Wellness Nurse (LPN) to provide direct nursing care to residents with a holistic approach. The role, led by the Health and Wellness Director, includes supervising care staff and ensuring compliance with standards.

The position is per diem with shifts from 7am to 3pm, 3pm to 11pm, or 11pm to 7am. A current LPN license and experience in long‑term care are preferred. Flexible scheduling and per diem benefits are offered.

Qualifications

• Graduate from an accredited School of Nursing with current LPN license.
• Experience in long-term care, assisted living, home health or hospital settings preferred.
• Knowledge of federal and state nursing regulations and practices.

Responsibilities

• Monitor health, safety, and well-being of residents.
• Supervise care staff and assist in medication administration training.
• Maintain clinical quality assurance per standards.
• Lead by example and uphold the company mission.
• Conduct thorough resident assessments.
