Job description

POSITION: HKMS 504 Coordinator: 2026-2027

Qualifications

Responsibilities

• Schedule 504 meetings including initial, annual, and transitional (this entails coordinating schedules with school staff and parents as well as sending invitations/procedural safeguards).
• Facilitate 504 meetings (and prepare minutes).
• Provide final 504 Accommodation Plan and meeting minutes to parents and staff within fourty-eight hours of the meeting.
• Maintain paper and electronic records of 504 plans (CT-SEDs), prepare meeting minutes, upload all related paperwork and maintain comprehensive records of the entire 504 population.
• Transfer Emergency Health Care Plans from school nurse e-file to a 504 Plan on CT-SEDs, mail/email home with meeting waiver to parents and provide with meeting waiver to classroom teachers. Note: This can be refused by parents/guardians.
• Notify Data Secretary within 24 hours of when a student qualifies or no longer qualifies for a 504 plan.
• One time meeting for the purpose of End of Year Reflection may be scheduled.

Stipend: $3,672 (1/2 paid in first paycheck in December, 1/2 paid in first paycheck in June)
