Job summary

The Florida Department of Health in Duval County seeks a Human Services Counselor III to assess HIV/AIDS clients, determine financial eligibility, and coordinate access to Ryan White/ADAP services across clinics and facilities.

You will document eligibility in CareWare and HMS, verify insurance and residency, and collaborate with community partners to ensure continuity of care for adults living with HIV/AIDS.

Qualifications

• 3+ years of experience in a medical office or medical setting.
• 1+ year with the Ryan White or ADAP program.
• Able to work 40 hours per week Monday through Friday.

Responsibilities

• Enroll newly diagnosed and active HIV+ clients for Ryan White/ADAP.
• Document eligibilities in CareWare and HMS.
• Verify insurance, income, and residency per policy.
• Coordinate referrals and continuity of care with community partners.
