Job summary
Care Resource Community Health Center in Miami, FL seeks a Client Support Assistant II in Social Services to welcome clients, educate them about services, and assist with housing and HIV/AIDS-related referrals. Qualifications
- Two years of related experience is required.

Responsibilities
- Welcome clients and provide orientation/education on agency services.
- Provide initial HIV/AIDS orientation and housing referrals.
- Assist with client intake, paperwork and applications for financial/housing eligibility.
- Develop individual client care plans and help schedule appointments.
- Maintain records and assist with chart reviews for QA.
- Communicate with clients about appointments and services; follow up with providers.
- Ensure HIPAA compliance and maintain referral network information.
- Participate in training and agency activities as required.
