Job summary

Rutgers University in Newark is seeking a Patient Navigator in the Department of Medicine- Infectious Disease within NJMS. You will provide medical case management, link clients to health care and support services, and ensure adherence to HIV/AIDS treatments.

The role involves initial assessment, service planning, and ongoing monitoring. Required: bachelor’s degree in health/public health/social work or related field and strong communication skills; two years’ experience.

Qualifications

• Bachelor’s degree in a health/public health/social work or related field required; two years’ related experience.
• Equivalent education, experience, and/or training may substitute for degree requirements.
• Must have strong written and verbal communication skills and Microsoft Office proficiency.

Responsibilities

• Provide medical case management linking clients to health care and support services.
• Coordinate and follow up to ensure timely access to treatments and adherence support.
• Develop and monitor comprehensive, individualized service plans.
• Conduct initial assessments and periodic re-evaluations; modify plans as needed.
• Orient clients to ID Clinic services and coordinate referrals to providers.
