Job summary

Qualifications

• High school diploma or equivalent; some college coursework preferred.
• Personal experience with a similar health condition valued.
• Strong communication, empathy and interpersonal skills.
• Ability to work independently and in a multidisciplinary team.
• Knowledge of Ryan White service delivery system is helpful.
• HIV 500/501 training certificates maintained and updated as required.

Responsibilities

• Facilitate individual peer counseling sessions and group support.
• Educate participants on resources and self-care aligned with goals.
• Maintain regular contact and monitor adherence to care plans.
• Collaborate with clinicians, case managers and other professionals.
• Engage in outreach and linkage activities for service access.
