Job summary

Mount Sinai Health System is seeking a Care Coordinator for the AIDS Center in Greenwich Village. The role supports patients in accessing HIV care, coordinating services, and adhering to treatment plans within a multidisciplinary team.

You will supervise Care Navigation Associates, conduct intake and ongoing assessments, and help educate patients to improve health outcomes. Strong communication in English (Spanish a plus) and EHR documentation are essential.

Qualifications

• Master's degree in Social Work, Mental Health Counseling or other clinical professional degree.
• 3+ years of case management experience.
• 2+ years of supervisory experience.
• Experience documenting in an electronic health record preferred.

Responsibilities

• Supervising Care Navigation Associates with oversight of service delivery, education, case management and patient progress.
• Enrolling eligible patients and conducting intake assessments and reassessments.
• Developing and updating patient-centered care plans in collaboration with the care team.
• Coordinating resources and referrals to ensure continuity of care and follow-up.
• Documenting encounters in the EHR and maintaining patient charts.
• Contributing to quality improvement and research initiatives.
