Qualifications

Responsibilities

• Provide culturally responsive HIV care management and care coordination.
• Assess needs and co-create health and well-being plans with clients.
• Facilitate communication between clients, families, and providers.
• Navigate resources and programs to bridge gaps in access to services.
• Advocate for clients and support goal achievement and self-identified outcomes.
• Conduct outreach and health education to clients and communities.
