Responsibilities

• Collaborates and coordinates with health care team to promote interdisciplinary treatment planning.
• Conducts intake interviews, psychosocial assessments to identify risk factors, and coordinates referrals.
• Assesses eligibility for health insurance plans and patient assistance programs, following up with services.
• Links patients to medical, behavioral health, housing, and social services across the care continuum.
• Monitors progress and documents through the EMR, ensuring adherence to treatment plans.
• Performs prior authorizations and collaborates with clinical staff for supporting documentation.
• Develops relations with community resources and participates in quality improvement initiatives.
• Assists with Outreach HIV/HCV rapid testing and counseling programs (certification).
