UNM Medical Group, Inc. is seeking a Medical Case Manager to join Truman Health Services in Albuquerque, New Mexico. This is a full-time day shift role focused on coordinating care for patients with chronic conditions and ensuring treatment adherence within a multidisciplinary team.

The ideal candidate will have strong communication, attention to detail, and solid computer skills. Required: education aligned with HIV populations experience; NM driver’s license; verification of education.

Qualifications

• High School Diploma or GED with 4 years HIV-related experience.
• Associates/CHW/Bachelors with 2 years HIV-related experience.
• BSW or MSW degree.
• Verification of education and licensure will be required.

Responsibilities

• Acts as liaison and advocate for services and care coordination.
• Develops goals and treatment plans with regular assessments.
• Coordinates and monitors care with multidisciplinary teams and community agencies.
• Ensures enrollment and recertification for Ryan White programs.
• Assists patients with applications for disability, SNAP, unemployment, insurance.
• Maintains confidentiality and comprehensive patient files.
