Job summary

St. Thomas Community Health Center is looking to hire a non medical case manager. The NMCM provides advice and assistance to empower clients to access a variety of necessary services to support medically related outcome measurements.

Qualifications

• High school or GED required.
• Ability to communicate effectively with clients, families and physicians and to complete documentation.

Responsibilities

• Complete comprehensive assessment of service needs by a non-medical case manager.
• Coordinate services required to support health outcomes.
• Periodic re-evaluation of client needs (housing, transportation, food, legal, emergency financial assistance).
• Encounter clients in various environments and inform them of Ryan White Part A services and agencies in NO EMA.
• Complete and develop Ryan White Plan of Care with the client and follow up on outcomes.
• Complete Ryan White billing forms and upload eligibility documents timely.
