Job summary

The State of Florida’s Department of Health – Clay County seeks a Social Services Counselor to support Ryan White program caseloads in a health‑care setting. You will conduct assessments, create care plans, and coordinate services to promote access to client-centered HIV care and treatment adherence.

Daily in-office or field work is required. The role emphasizes collaboration with partners, data entry in CAREWARE/HMS, and meeting HRSA guidelines while maintaining client confidentiality.

Qualifications

• Bachelor’s degree in psychology, social work, or human services or equivalent education/experience.
• Six months of case management experience or applicable substitute.
• Valid State Driver's License required.

Responsibilities

• Complete initial assessment and develop an individualized care plan.
• Re-enrollment and documentation according to HRSA standards.
• Coordinate medical referrals and follow-up appointments.
