Job summary

Community Healthcare Network (CHN) operates Health Home Plus (HH+) to provide intensive care management for PLWH, addressing medical, behavioral health, and social service needs in the community. The program emphasizes adherence to HIV care, viral suppression, and coordinated, patient-centered planning.

HH+ staff conduct thorough assessments, build individualized care plans, and perform regular home visits, ensuring four core services per month, including two face-to-face interactions with

Qualifications

• Masters or Bachelor's Degree in Human Services required; one year experience with target populations defined as HIV, mental illness, homelessness or substance abuse.
• Associates Degree with at least two years of relevant experience in the same target populations.

Responsibilities

• Provides direct service to a caseload of approximately 15-20 patients.
• Conducts and documents initial biopsychosocial assessments.
• Face-to-face assessments and plan-of-care updates in patients' homes.
• Provides crisis intervention and health education services.
• Offers HIV prevention, risk reduction, and treatment education.
• Collaborates on care plans with providers and patients for goal achievement.
• Performs home visits and maintains contact per program standards.
• Coordinates care with internal and external providers through care conferencing.
