Qualifications

• Bachelor's degree as above with practicum in case management or five years of experience in human services may substitute for degree.
• Strong interpersonal skills and ability to handle confidential information.
• Ability to interact effectively with consumers, administration, faculty, and staff.
• Compliant with BHCC Standards and AIDET awareness.

Responsibilities

• Coordinates the interdisciplinary plan of care for a patient.
• Evaluates eligibility for case managed services.
• Acts as liaison between patient, family and healthcare personnel to ensure timely care.
• Enrolls HIV positive clients in early intervention services.
• Assists clients in goal planning and setting short/long-term goals.
• Maintains and updates client intake and care plans every six months.
• Monitors service delivery and client adherence to care plans.
• Provides individual counseling and crisis intervention as needed.
• Advocates for entitlements and connects clients to social service supports.
• Participates in client-specific case conferences.
• Monitors and resolves access issues and coordinates with other agencies for continued services.
