Job summary

CAN Community Health in Miami Beach is seeking a Medical Case Manager I to coordinate and deliver comprehensive case management services for clients affected by HIV, Hepatitis C, STI's, and other infectious diseases. This full-time, day-shift role involves eligibility screening, care planning, resource linkage and collaboration with healthcare teams to promote health outcomes.

Qualifications

• Bachelor’s degree in social work, nursing, public health, or related field.
• One year of HIV prevention/intervention experience.
• Must be able to operate a motor vehicle and have valid driver's license.
• Must pass Level I and Level II Background checks.

Responsibilities

• Screen clients for program eligibility and conduct biannual reassessments.
• Verify insurance coverage and coordinate required pre-authorizations.
• Develop, implement, and update individualized care plans with patient collaboration.
• Act as liaison between patients and the care team to address identified needs.
• Collaborate, develop, implement, monitor and evaluate the case management process.
• Perform comprehensive assessments to identify health, mental health, social support, and financial needs.
