Legacy Community Health in Houston, TX is seeking a Medical Case Manager for the Ryan White program. You will help HIV-positive patients access care, navigate services, and maintain engagement through psychosocial support and advocacy.

The role emphasizes collaboration with Legacy providers, patients, families, and community partners to promote health, independence, and coordinated, culturally sensitive care across locations.

Qualifications

• One year of case management experience with HIV patients.
• Excellent communication and customer service skills.
• Bilingual English/Spanish is required.

Responsabilités

• Identify and screen HIV+ individuals referred for social services.
• Provide referrals and linkage to medical, psychosocial, and basic needs services.
• Advocate to reduce service gaps and remove barriers to care.
• Assist patients who have been lost to care to return to medical services.
• Maintain documentation per program standards and EHRS requirements.
• Travel between Houston and Beaumont locations as needed.
• Participate in training and quality improvement activities.
