Job summary

Vivent Health is seeking a dedicated Case Manager to join our collaborative healthcare team in delivering comprehensive, compassionate care. You will assess medical and psychosocial needs, coordinate services, and advocate for clients to ensure access to needed resources and support.

As part of an interdisciplinary group, you will educate clients, document activities, and participate in crisis intervention while upholding confidentiality and ethical standards.

Qualifications

• Bachelor’s degree in Social Work, Psychology, or a related field is required.
• Strong interpersonal and communication skills; empathy and cultural awareness.
• Ability to learn multiple databases and software systems.

Responsibilities

• Works scheduled hours within the clinic setting as part of the internal care team.
• Conduct thorough assessments of clients' medical and psychosocial needs to develop individualized case management plans.
• Coordinate access to medical services and social supports to enhance client well‑being.
• Assist clients in accessing community resources, social services, and insurance coverage.
• Advocate for clients within the healthcare system to ensure needs are met.
• Provide crisis intervention and maintain accurate documentation of activities.
• Collaborate with physicians, nurses, and mental health professionals to achieve optimal outcomes.
