Job summary

Vivent Health is seeking a Case Manager to join our interdisciplinary healthcare team in Madison, WI. You will conduct comprehensive assessments, coordinate medical services, and support clients to access community resources and insurance coverage.

You will advocate for clients, provide crisis intervention when needed, and maintain compliant documentation while collaborating with physicians, nurses, and behavioral health staff to achieve optimal outcomes.

Qualifications

• Bachelor's degree in Social Work or related field is required.
• Experience in case management preferred.
• Valid Government Issued Driver’s License required.
• Familiarity with coordinating and managing patient care plans.

Responsibilities

• Conduct comprehensive assessments of clients' medical and psychosocial needs to create individualized care plans.
• Coordinate access to medical services with healthcare providers for holistic patient care.
• Perform eligibility assessments and gather documentation for grant funded services.
• Assist clients in accessing community resources and insurance coverage.
• Advocate for clients within the healthcare system and support crisis intervention.
