Legacy Community Health in Houston invites a Ryan White Medical Case Manager to help HIV-positive patients access care, resources, and encouragement. You will work with Legacy providers, patients, families, and community partners to deliver compassionate psychosocial services addressing medical needs and daily challenges.

This role requires a Master’s in social work, current Texas LMSW license, and at least one year of HIV case management experience.

Qualifikationen

• Master’s degree in social work is required.
• Current Texas LMSW is required. Maintain licensure as defined by Texas boards.
• One year of case management experience with people living with HIV or related conditions is required.
• Bilingual English/Spanish is required with fluency in both languages.

Aufgaben

• Identify and screen HIV+ individuals referred for social service needs.
• Provide referrals and assistance with linkage to medical, psychosocial, and basic needs services.
• Advocate to decrease service gaps and remove barriers to services.
• Assist patients who have been lost to care in returning to medical services.
• Document services in EHRS within 72 hours and participate in QA/QI activities.
