Job summary

AIDS Healthcare Foundation in the United States seeks a Medical Care Manager to support Ryan White clients through comprehensive case management, needs assessments, action/care plans and ongoing education about diagnoses and medications.

You will collaborate with healthcare center teams, arrange referrals for housing, food, and services, and help clients access resources while aligning with HAB measures and the Ryan White contract.

Qualifications

• Bachelor’s or Master’s degree in Social Science, Social Work, or Human Services.
• Minimum 1 year of Case Management experience preferred.
• Knowledge of Ryan White Services preferable.

Responsibilities

• Conduct Needs Assessments and develop Action/Care Plans.
• Provide ongoing education to patients about diagnoses and medications.
• Arrange referrals to housing, food, and other supportive services.
• Collaborate with providers and center team to achieve client goals.
