Job summary

Qualifications

Responsibilities

• Provides client-centered services linking clients with health care, psychosocial, and other services for coordinated access to care.
• Prepares confidential and non-confidential correspondence professionally.
• Inputs data into databases accurately and timely.
• Makes appointments and referrals for clients as needed.
• Maintains filing systems and required information promptly.
• Complies with TDH standards for charting and case management.
• Records client information securely and appropriately.
• Assists supervisor with programmatic reports and timely delivery.
• Communicates with supervisor about programmatic and client information.
• Engages with clients and community partners professionally to avoid service duplication.
