Job summary

El Camino Health in Mountain View, CA seeks an HIM Specialist I to support daily validation and reconciliation of patient documentation, chart preparation and scanning for the facility’s legal medical record.

Responsibilities include chart assembly and validation, ER chart analysis, scanning with QA, accurate indexing, processing care planning documentation and releasing information requests, with emphasis on accuracy, privacy, and timely workflows.

Qualifications

• High school diploma or equivalent required.
• Basic computer skills including Microsoft Office (Outlook, Word, Excel).
• Experience in health information management or healthcare setting preferred.
• Ability to work independently and in a team within a fast-paced environment.
• Knowledge of medical terminology preferred.

Responsibilities

• Validate and reconcile patient documentation for the medical record.
• Chart assembly and validation; analyze loose sheets.
• ER chart analysis and QA of scanned images.
• Index patient information accurately and timely.
• Process requests for care planning documentation and release of information.
• Validate insurance/payor requests for release of information; create/scan requests.
• Assist patients in the lobby with record requests.
• Prepare records for off-site storage and maintain chart tracking.
