El Camino Health seeks an HIM Specialist II to serve as the primary contact for processing legal medical records and coordinating patient access to health information. The role involves interviews with patients by phone or in person, ensuring accurate documentation and timely retrieval of records across multiple sites.

You will support birth recording, release of information, and MPI maintenance, while collaborating with clinical and non-clinical staff to optimize the patient experience and data

Qualifications

• High school diploma or equivalent; associate degree preferred.
• Experience in acute care or healthcare settings with chart analysis, birth recording, or release of information.
• Proficient with Microsoft Office and EMR systems.

Responsabilités

• Process daily patient medical record documentation for chart validation and scanning into the legal medical record.
• Assist with analysis to ensure record completeness per CMS/Joint Commission and hospital bylaws.
• Coordinate with practitioners for chart completion and Title 22 compliance.
