Job summary

Advocate Health in Charlotte, NC seeks a full-time Document Management Specialist in the HIM Ops department to analyze and classify clinical documentation, manage deficiencies, and ensure accurate coding and billing.

Responsibilities include reviewing and uploading to Epic, supporting Health Maintenance directives, ensuring compliance with TJC, DNV, and CMS, and coordinating with IT and coding teams to meet quality metrics.

Qualifications

• High School Diploma or GED Equivalent.
• 1–2 years of healthcare experience preferred.
• Experience with Epic, OnBase and EHR systems.
• Strong attention to detail and data integrity.
• Knowledge of HIPAA and PHI handling.

Responsibilities

• Analyze documentation needs and classify clinical records for services provided.
• Assign, edit and track medical record deficiencies to support coding and billing.
• Review, upload and process to Epic and Health Maintenance directives.
• Ensure compliance with Joint Commission, DNV and CMS regulations.
• Collaborate with leaders, IT, coding and HIM Optimization to resolve issues.
• Validate deficiencies and chart completion in the EHR and notify clinicians.
• Verify presurgical H&Ps and other documents via OnBase and upload to the EHR.
• Coordinate training for Epic Connect clinics moving to EPIC.
