Job summary

Monument Health is seeking an HIM Coder II to accurately code hospital outpatient, surgical, and professional services in accordance with Official coding guidelines and payor standards.

The role requires coding accuracy, knowledge of ICD-10, CPT, and HCPCS, and active participation in coding education. Strong attention to detail and ability to query providers for missing documentation are essential.

Qualifications

• Education - High School Diploma/GED Equivalent
• Experience - 1+ years of Coding Experience
• Certifications - CCS (AHIMA) or CPC-A (AAPC) preferred

Responsibilities

• Review and abstract information in the medical record to accurately code for that episode of care.
• Code hospital and professional services by accurately assigning ICD-10, CPT and HCPCS codes for appropriate billing.
• Apply knowledge of medical terminology, anatomy and physiology and coding guidelines to select correct codes.
• Maintain knowledge of NCCI, LCDs/NCDs, MUEs and Medicare guidelines.
• Participate in coding education and training.
• Provide feedback to physicians and staff regarding coding guidelines.
• Query providers for missing documentation.
• Resolve payor edits and denials and coordinate with compliance and billing.
• Navigate encoder and grouper software and coding resources.
• Maintain coding productivity and accuracy standards.
• Assist with patient audits and obtain necessary information.
• All other duties as assigned.
