Job summary

The Harris Center for Mental Health and IDD is seeking a High Fidelity Wrap around Facilitator in Houston, TX, to provide intensive case management using wraparound principles. The role involves extensive travel to client homes and field locations, collaborating with DFPS and Harris County Juvenile Courts, and ensuring treatment plans are written, authorized, and aligned with eligibility criteria.

Qualifications

• Bachelor’s Degree in Human Services field is required.
• Minimum of 1 year experience in mental health.
• Ability to work collaboratively in a team.

Responsibilities

• Collaborates with DFPS investigators and social workers to develop the high-fidelity wraparound plan.
• Performs clinical assessments to identify service needs and progress with family treatment goals.
• Formulates family wraparound plans and ensures LPHA-signature for consumers on caseload.
• Ensures services meet eligibility criteria and are properly authorized.
• Provides skills training and psychosocial rehabilitation addressing mental health symptoms.
• Responds to crises to reduce symptoms and avoid hospitalization.
• Educates families on youth needs and coordinates access to care and services.
• Monitors progress toward treatment goals and adherence to plans.
• Maintains accurate documentation and adheres to regulatory guidelines.
• Participates in team meetings and adapts to changing requirements.
