Job summary

PrimaryOne Health is hiring an HFA Home Visit Program Manager for our Columbus health centers. You will recruit and enroll families, supervise home visits, and ensure comprehensive care in line with HFA and Growing Great Kids curricula.

The role requires a Master’s or Bachelor’s with relevant experience, leadership abilities, and collaboration across healthcare teams. Occasional travel and a strong commitment to quality improvement are essential.

Qualifications

  • Master’s degree in public health or human services or related field.
  • Bachelor’s degree in related fields and 3 years of relevant experience; or less than bachelor’s with commensurate HFA experience.
  • 2–3 years’ experience in maternal/child health, public health, home visiting or equivalent combinations of education/experience preferred.

Responsibilities

  • Assists in recruitment and enrollment of women and families eligible for the HFA program.
  • Acts as a family support specialist to ensure continuous and comprehensive care for caseloads.
  • Assists in staff selection, orientation, training, and records reviews; maintains data collection systems.
  • Implements a quality assurance plan and monitors performance of Family Support Specialists.
  • Maintains perspective, supports staff development, and fosters reflective practice to enhance service quality.
  • Liaisons with other agencies, monitors contracts, and supports program development.
  • Provides and supervises Home Visits to support onboarding and caseload capacity.
  • Collaborates with health care team including providers, pharmacists, dietitians, behavioral health staff and medical assistants.
  • Ensures adherence to HFA, Ohio Dept of Health Help Me Grow, and PrimaryOne Health policies and standards.
  • Documents all activities promptly; attends meetings and participates in Quality Improvement.
  • Regular travel required; maintains professionalism and attention to detail.

PrimaryOne Health is currently seeking an HFA (Healthy Families of America) Home Visit Program Manager for our Columbus health centers.

Essential Job Responsibilities
  • Assists in recruitment and enrollment of women and their families eligible for the HFA program
  • Works as a family support specialists as need be and ensures continuous and comprehensive care for patients on caseload in adherence to HFA and Growing Great Kids curriculum.
  • Assists in the selection of staff, participates in the orientation and training, conducts record reviews, assists in or maintains the data collection system, implements a quality assurance plan, and monitors the performance of Family Support Specialists.
  • Helps staff maintain perspective, evaluate their own performance, increase personal and professional development, learn and practice new strategies to effectively work with families, and develop reflective capacity, and ultimately enhance the quality of services families receive.
  • Acts as a liaison with other agencies and monitor contracts as well as ongoing program development.
  • Able to provide and supervise Home Visits as needed to support onboarding new staff and support the program’s caseload capacity.
  • Works collaboratively with the health care team including the providers, pharmacists, dieticians, behavioral health specialist, other nursing staff and medical assistants.
  • Responsible for adherence to policies, procedures, guideline and standards of HFA, Ohio Department of Health Help Me Grow, and PrimaryOne Health.
  • Completes documentation of all professional activities, contacts and observations promptly and appropriately.
  • Attends internal and community meetings as assigned and participates in Quality Improvement activities.
  • Regular travel required.
  • Attendance, promptness, professionalism, the ability to pay attention to detail, cooperativeness with co-workers and supervisors, and politeness to customers, vendors, and patients.
  • Stays informed of current health care developments within the field of maternal/child health, public health, and home visitation
  • Other duties or special projects as assigned.

Successful candidates must have aMaster’s degree in public health or human servicesor fields related to working with children, or Bachelor's degree in these fields and 3 years of relevant experience, or less than a bachelor’s degree but with commensurate HFA experience. 2-3 years’2-3 years’ experience in maternal/child health, public health, home visiting or equivalent combinations of education/experience preferred.Experience with Healthy Families America preferred. Leadership experience preferred.Administrative experience in human services or related fields, including experience in quality assurance and continuous quality improvement.
