Job summary

Qualifications

  • LPN coursework completion and licensure strongly preferred.
  • Certification as Medical Assistant or Community Health Worker or a degree in public health will be considered.
  • At least one year of experience working in a medical office strongly preferred.
  • Certified Medical Assistant, LPN, CHW, or related healthcare field with two or more years of related experience.
  • Strong communication and organizational skills.
  • Proficient in PC use and multiple applications including Microsoft Office, and Electronic Health Records (EPIC knowledge preferred).

Responsibilities

  • Using patient lists generated within EPIC or via Excel, review patients charts to identify pregnant women that are getting appropriate prenatal care.
  • Conduct outreach calls to patients and other medical offices involved in the care of our pregnant patients coordinate care.
  • Ensure patients are getting medications and adherent to medications. Coordinate with other HOFH departments (pharmacy, provider, etc.) and other medical offices involved in the patient to ensure medication adherence.
  • Identify barriers to receiving needed medical care. Connect the patient with resources and work with the patient and other HOFH departments (CHWs, referrals, pharmacy, provider, etc.) to coordinate assistance for patients.
  • Serve as a contact, advocate, and educational resource for patients and families. Educate patients/family members on the care plan outlined by medical providers.
  • Participate in other Quality Improvement activities.

Position Type: Full Time

Salary Range: $24.00 - $28.00 Hourly

Summary: The Pregnancy Care Coordinator functions as a clinical support staff responsible for helping to coordinating care for Heart of Ohio Family Health patients receiving prenatal care. The position will assist with monitoring a cohort of pregnant women to ensure that they are attending appointments at our clinic, attending MFM appointments, getting needed procedures (example: NSTs), getting needed medications, and other more.

Developing, implementing, and evaluating care plans for patients requiring intensive care management, with a focus on chronic conditions such as diabetes and hypertension. Working collaboratively with a multidisciplinary care team—including providers, nurses, dietitians, pharmacists, behavioral health clinicians, and community health workers—the RN Clinical Care Coordinator ensures that high-risk patients receive comprehensive and coordinated care. In addition to direct patient care in the clinic, the RN Care Coordinator will also support quality improvement initiatives and projects, administer vaccines and injectable medications, and perform certain diagnostic procedures (e.g., spirometry, diabetic retinal imaging, etc.).

Work Schedule: Monday through Friday during standard business hours. Times are subject to change due to business necessity.

Facility Environment:

Heart of Ohio Family Health operates in multiple locations, in the Columbus, OH area. All facilities have a medical office environment with front-desk reception area, separate patient examination rooms, nursing stations, pharmacy stock room, business offices, hallways and private toilet facilities. All clinical facilities are ADA compliant.

This position’s primary work area is the clinical portion of the facility; ie, patient examination rooms, hallways, and other clinically assigned areas.

The clinical area is:

  • kept at a normal working temperature
  • sanitized daily
  • maintains standard environment furniture with adjustable chairs
  • maintains standard office equipment; ie, computer, copier, fax machine, etc. at a normal working height
Qualifications

Job Qualifications (Experience, Knowledge, Skills and Abilities)

Education / Experience / Licensure:

LPN coursework completion and licensure strongly preferred. Candidates who have completed a certification as a medical assistant, certification as a community health worker, or a degree in public health will be considered. At least one year of experience working in a medical office strongly preferred.

  • Certified Medical Assistant, LPN, Community Health Worker, or related healthcare field from an accredited college or university and two or more years of related experience.
  • Excellent communication, organizational, and interpersonal skills.
  • Proficient in PC use and multiple applications including Microsoft Office, and Electronic Health Records (EPIC knowledge preferred). Basic experience with Microsoft Excel data entry preferred.
  • Understanding of client confidentiality issues, HIPPA compliance and regulations.
