Job summary

Recovery Network of Programs seeks a dedicated case manager to support HERO participants. You will conduct sessions, assess housing and health needs, and help Consumers complete HERO assessments and recovery plans.

You will advocate for housing resources, liaise with landlords and service providers, and maintain progress notes to track progress toward self-sufficiency. The role involves in-person, office and home visits, collaboration with the team, and participation in program meetings.

Qualifications

  • Bachelor's degree in the behavioral sciences and two years experience in counseling preferred, or equivalent combination of education and experience.

Responsibilities

  • Conduct individual sessions with clients, including in-person, office and home visits to assess medical, psychological and social factors.
  • Assist Consumers with HERO Participant agreement and service needs.
  • Complete HERO Case Management Assessment within 30 days of acceptance and every 6 months thereafter.
  • Develop and implement recovery plans addressing housing, income, benefits and health needs.
  • Maintain progress notes and document housing, financial, and health information.
  • Advocate for housing resources and coordinate with landlords and service providers.
  • Participate in meetings and supervision; maintain professional relationships.

SUMMARYIn accordance with RNP's Mission Statement, Philosophy of Client Care, and Code of Ethics as stated in the Personnel Policies and under the supervision of the Department Director or Coordinator, assist in the provision of quality care for individuals served by performing the following duties.HOURS: Monday - Friday 8:00 am - 4:00 pm with flexibilityCORE FUNCTIONSConduct individual sessions with clients including in-person, office and home visits to assesses and records medical, psychological, and social factors contributing to client's situation and evaluates these capacities.Assist the Consumer with completing the HERO Participant agreement and identifying service and support needsComplete HERO Case Management Assessment for each Consumer within thirty (30) days of the Consumer’s acceptance into the HERO program and every six (6) months thereafter to help the Consumer explore and identify personal strengths, identify community and recovery resources and supports to enable the Consumer to increase self-sufficiencyDevelop and implement a recovery plan with each Consumer that addresses identified needs in areas such as: housing and lease, arrears and debts, income and benefits, support services and resources, health, parenting and child services. Such plans shall contain goals identified by the Consumer and shall provide detailed information on goals, objectives, tasks, and interventions, and shall identify the person responsible and time frames for accomplishmentAdhere to all standards as it relates to documentation requirements.Assist Consumers with services and supports that enhance independent participation in social, interpersonal, family, and community activities.Assist consumers with developing self-advocacy skills for the purpose of accessing natural supports.Serve as an advocate to ensure that Consumers are linked to services which meet the needs identified in their consumer recovery plan and that are culturally competent.Provide liaison services between Consumer, landlords, and other service providersProvide community education on tenant rights to HERO participants through informational materials, including, but not limited to, pamphlets, referrals to legal servicesRefer Consumer to employment servicesProvide advocacy for equal access to housing resources to HERO participants through informational materials, including, but not limited to, pamphlets, referrals to legal services; andProvide resource information to consumers, service providers, and community groups on housing related issuesParticipate in the DMHAS HERO quarterly meetings either virtually or in-person, as scheduled by DMHASEnsure that Consumers from all programs complete the DMHAS annual Consumer Satisfaction Survey and compile the results for submission to DMHAS or its designee by June 30th each contract yearMaintain timely progress notes which record and monitor client progress primarily with regard to housing but may include finances, employment, nutrition, basic needs, physical health, mental health and substance abuse related issues.Maintain a current list of local landlords, realtors, and property management companies with apartments.Attend and be prepared for case management and staff meetings.Participate in individual supervision and prepare supervision agenda.Assumes all other appropriate duties as deemed necessary by the supervisor.CORE COMPETENCIESBachelor's degree in the behavioral sciences and two (2) years experience in counseling preferred; or equivalent combination of education and experience. Co-occurring competencies and or credentials are preferred.Commitment to the mission of ending homelessness.Knowledge of principles and techniques of social service case management services.Knowledge of the physical, psychological, social and economic effects of homelessness, substance abuse, physical and emotional illness, crisis intervention.Ability to work with diverse populations. Ability to work independently or with a group. Assertively link individuals to housing and community resources. Ability to engage local landlords, explain subsidies process and advocate for landlord participation. Ability to coordinate multiple tasks simultaneously. Highly motivated, organized, creative with excellent interpersonal skills. Bilingual/Bicultural preferred.Must be able to work effectively with populations who have experienced vulnerability, trauma, economic poverty, incarceration, substance use, developmental delays, compromised mental wellness, brain injuries, literacy and numeracy issues, and/or other conditions or situations that have impacted housing stability.Ability to gather, analyze and evaluate information pertinent to client care.Knowledge of community resources.Considerable knowledge of federal and state laws and regulations regarding client confidentiality.Ability to establish and maintain cooperative professional relationships.Excellent communication skills.Ability to project a professional image.CERTIFICATES, LICENSESIncumbents may be required to secure professional certification and/or license.Valid Connecticut driver's license is to be required.
