Catholic Charities of the Archdiocese of St. Louis, St. Patrick Center, is hiring a Hero Program Case Manager.

The Case Manager will use wrap-around philosophy to assist clients in transitional housing and coordinate individualized services under the HERO program. Responsibilities include meeting with clients, tracking the ISS/Treatment Plan of Care, collaborating with providers in home and community settings, and documenting all support activities.

Job description

Catholic Charities of the Archdiocese of St. Louis, St. Patrick Center, is hiring a Hero Program Case Manager. The Case Manager, as part of the Housing, Employment and Recovery Opportunities (HERO) program, will utilize wrap-around philosophy and techniques to assist clients in transitional housing. The case manager will meet with clients to maintain family and natural supports in the community, collaborate with clients and track the success of the Individual Service Strategy (ISS)/Treatment Plan of Care, coordinate individualized services and the involvement of providers working in the home and community setting and document all support related to activities and services
