AdventHealth is seeking a transplant care coordinator to organize referrals, evaluate patients for advanced therapies, and educate families about treatment and post‑care. You will liaise with surgeons, cardiologists and transplant teams to ensure comprehensive care throughout the patient journey.

The role emphasizes accurate documentation, adherence support and ongoing communication with patients, families and clinicians in a dynamic hospital environment in Orlando, FL.

Qualifications

• Ability to communicate verbally and in writing in English with physicians, patients and staff.
• Strong interpersonal and organizational skills for a high‑volume clinical setting.
• Autonomous, self‑directed while collaborating with professionals in high‑complexity situations.
• Self‑motivated with the ability to multi‑task and adapt to changing demands.
• Mature judgment and handling of confidential information per Florida Hospital guidelines.
• Knowledge of clinical documentation, exam procedures and protocols.
• Strong communication and organizational skills.
• Basic clerical skills and ability to type 20 WPM.
• Bilingual (English required/Spanish preferred).
• Medical terminology knowledge is preferred.

Responsibilities

• Organizes and schedules appointments, tests and consultations with transplant teams and specialists.
• Assesses patient eligibility for advanced therapy by reviewing records and preliminary tests.
• Coordinates referrals and patient evaluations for transplant or donor programs.
• Provides education to patients and families about treatment, risks, benefits and post‑care.
• Maintains accurate records of patient interactions, assessments and care plans.
• Acts as liaison between patient, family and healthcare team to ensure clear communication.
• Monitors progress post‑treatment or post‑transplant, schedules follow‑ups, manages complications.
• Ensures adherence to prescribed medications and educates on side effects.
• Participates in quality improvement to enhance patient care and outcomes.
• Stays informed about transplant medicine advancements and applies evidence‑based practice.
