Job summary

The Johns Hopkins University in Baltimore, MD is seeking a Sr. Clinical Nurse Case Manager to provide medical and non-medical case management within a multidisciplinary HIV care team.

The role includes counseling for patients on PrEP and nPEP, STI testing, and prevention interventions aligned with clinic protocols. Under the supervision of the Clinical Nurse Manager, you will coordinate care, monitor treatment adherence, conduct psychosocial assessments, and participate in quality improvement

Qualifications

• Must be a Maryland-licensed registered nurse.
• Minimum of two years of nursing experience; additional education or experience may substitute as allowed by policy.

Responsibilities

• Provide case management for HIV care patients, monitor and educate about medications and lab results.
• Conduct psychosocial assessments and develop individualized care plans.
• Provide HIV treatment and ART adherence counseling and coordinate with pharmacies.
• Coordinate long-acting HIV therapies and PrEP/nPEP management.
• Review results, triage patients, and document all care in the medical record.
• Lead/participate in protocol development and quality improvement activities.
