Under the supervision of the Project Manager, the Viral Hepatitis Case Manager provides patient-centered support to help clients overcome barriers to hepatitis treatment, HIV linkage to care, and access to needed services. This role coordinates referrals, supports continuity of care, conducts outreach, and contributes to program development, staff training, and quality improvement efforts.
Salary Range: Minimum 19.25/hour - Maximum 31.80/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
