Job summary

Native American Rehabilitation Assoc. of the NW seeks a Hepatitis C Nurse Case Manager (RN) for the Indian Health Clinic in Portland, OR.

This full-time, day-shift role offers a $1,000 sign-on bonus and a schedule of 8:00 AM–5:00 PM, Monday–Friday with occasional evenings or weekends. The RN will conduct testing, provide patient education, coordinate treatment plans, and ensure culturally responsive, trauma-informed care across clinical and mobile outreach settings, with travel between sites as

Qualifications

• Bachelor of Science in Nursing required.
• Registered Nurse licensure in Oregon and Washington required.
• Ability to work independently and as part of an interdisciplinary team.
• Ability to work occasional evenings and Saturdays.
• Proficiency in Electronic Health Records (EHR) systems and documentation standards.

Responsibilities

• Provides nurse case management for Hepatitis C testing and treatment, coordinating care plans and adherence.
• Educates patients on transmission, prevention, treatment options, and harm reduction.
• Monitors treatment progress, updates care plans as needed.
• Acts as patient advocate and liaison among patients, families, providers, and community partners.
• Collaborates with staff to deliver integrated, patient-centered services.
• Delegates tasks to medical assistants and supports their ongoing training.
• Performs patient intake, vital signs, risk screening, phlebotomy, and point-of-care testing.
• Participates in mobile outreach to engage high-risk populations.
• Maintains accurate documentation in the EHR.
