Job summary

Paycom - ATS is seeking a Hepatitis C Case Manager to ensure timely access to care, treatment, and medications for patients across Ohio and Pennsylvania. You will liaise with Hepatitis C Providers, Nurses, pharmacists, insurers, and assistance programs to resolve barriers and coordinate benefits and medication access.

You will provide patient-centered, trauma-informed support within HIPAA-compliant Epic workflows, focusing on thorough documentation and proactive communication.

Qualifications

• High school diploma or equivalent required.
• Healthcare-related case management or navigation experience preferred.
• Strong organizational and communication skills with attention to detail.
• Ability to manage multiple patients and priorities.
• Willingness to learn Epic, Salesforce, payer portals, and pharmacy systems.

Responsibilities

• Verify insurance eligibility and coverage, and assist with enrollment.
• Coordinate medication access and monitor prior authorizations and copays.
• Facilitate patient assistance programs and financial advocacy.
• Maintain accurate Epic documentation and program tracking.
• Engage patients through proactive outreach and care coordination.
