• Monitor treatment progress, track outcomes, and update care plans as needed.
• Advocate for patients and liaise with families, healthcare providers, and community partners.
• Collaborate with medical providers, pharmacists, MAs, Behavioral Health, and outreach staff to deliver integrated services.
• Delegate tasks to medical assistants and support ongoing training.
• Perform tasks such as patient intake, vital signs, risk screening, phlebotomy, and point-of-care testing for HCV/HIV/STIs.
• Participate in mobile outreach and community-based services to engage high-risk populations.
• Maintain accurate documentation in EHR and required systems.

Position Overview

Title: Hepatitis C Nurse Case Manager - RN

Locations: Indian Health Clinic - 703 NE Hancock St., Portland, OR 97212; Wellness Center - 12360 E. Burnside St., Portland, OR 97233

Schedule: This is an in-person position, Monday through Friday, 8:00am – 5:00pm, with occasional evenings or weekends required.

Wage Range: $50.00 – $55.00 hourly, non-exempt

What You Will Do

• Provides nurse case management services for patients undergoing Hepatitis C testing and treatment, including coordination of care plans, medication adherence support, and follow-up.
• Conducts comprehensive patient education on Hepatitis C transmission, prevention, treatment options, and harm reduction strategies.
• Monitors treatment progress, tracks outcomes, and updates care plans as needed.
• Serves as a patient advocate and liaison between patients, families, healthcare providers, and community partners to ensure seamless and compassionate care.
• Collaborates with medical providers, pharmacists, medical assistants, Behavioral Health Consultants, and outreach staff to deliver integrated, patient-centered services.
• Delegates appropriate tasks to medical assistants and supports their ongoing training.
• Performs medical tasks as needed, such as patient intake, vital signs, risk screening, phlebotomy, and point-of-care testing for HCV, HIV, and STIs following the NARA NW MA Competency Guidebook.
• Participates in mobile outreach and community-based services to engage high-risk and underserved populations.
• Maintains accurate, timely, and compliant documentation in the Electronic Health Record (EHR) and other required systems.
• Demonstrates tact, diplomacy and compassion; acts as a patient advocate and provides support in emergency and stressful situations.
