Qualifications

• Bachelor’s degree in Social Work, or similar field from an accredited college or university preferred.
• Experience performing community outreach, preferably about health or social services programs.
• Ability to maintain confidential medical information.

Responsibilities

• Assess individual needs and connect clients to services.
• Educate clients on hepatitis C and overdose prevention.
• Schedule appointments and assist in accessing supportive services.
