Job summary

CVS Health delivers a unified health experience by connecting patients, providers, and services. We seek an Analyst, Outreach Coordinator to lead HEDIS improvement work, facilitate provider meetings, and guide education to close care gaps across the country.

You will collaborate with internal partners and external providers to address care gaps, answer questions about HEDIS processes, and support outreach that enhances data collection and quality measures.

Qualifications

• 3+ years' health care experience in any setting.
• 1+ years Medicare experience and/or 1+ years HEDIS medical record abstraction experience.
• 1+ years of member/provider education targeting gaps in care.
• Experience with productivity metrics in a healthcare environment.
• Ability to work independently and with standard corporate software.
• Strong communication skills.

Responsibilities

• Subject Matter Expert in HEDIS improvement work.
• Interact with providers/members to improve healthcare outcomes.
• Support provider engagement and participate in workgroups.
• Deliver quality information via written and verbal communication.
• Identify and implement process improvement opportunities.
• Map out and execute provider outreach work plans.
• Support complex relationships with internal and external constituents.
• Consult with stakeholders on education and process improvements.
• Assist in gaps in care, digital solutions, and medical record collection discussions.
• Collaborate with providers to enhance collaboration with Aetna.
• Support HEDIS quality initiatives and regulatory requirements.
• Assist in medical record capture for HEDIS reviews.
