Job summary

Advocate Health in North Carolina is seeking a Heart Transplant/VAD Coordinator to support the Advanced Cardiac Care and Transplant program. You will coordinate evaluation, hospitalization and follow-up for heart transplant and VAD patients, educate families, act as patient advocate, and liaise with physicians, HMOs and the organ procurement network.

The ideal candidate is a BSN-educated RN with at least five years in cardiology or cardiothoracic surgery, NC license, and CPTC preferred.

Qualifications

• BSN from an accredited nursing program and NC RN license.
• Minimum five years' experience in cardiology and/or cardiothoracic surgery.
• Current NC RN license; CCT or CPTC preferred.

Responsibilities

• Coordinate evaluation and data to support transplant and VAD programs.
• Educate patients and families about transplantation and post-op care.
• Communicate with referring physicians, HMOs and insurance providers.
• Maintain data and participate in program development and quality improvement.
• Coordinate preoperative prep, intra-operative and postoperative care as needed.
• Provide staff education and ensure adherence to plan of care.
