• Develop an individualized plan of care with patient/family and health care team.
• Implement and document interventions to meet patient outcomes.
• Evaluate progress and adjust plan as needed.
• Ensure charting and event reporting is completed.
• Maintain up-to-date nursing knowledge and skills.
• Promote patient and staff safety and assist with care equipment as needed.
• Collaborate with team from admission to discharge per plan of care.
