Job summary

Responsibilities

• Assesses patient status through interview, observation and physical assessment.
• Develops an individualized plan of care with patient/family and health care team.
• Uses interventions to achieve expected patient outcomes.
• Documents patient progress and re-evaluates plan of care.
• Collaborates with health care team from admission to discharge.
• Promotes patient and staff safety and professional development.
