Qualifications

• Current RN license in the state of practice.
• 2–3 years of nursing experience required.
• BLS certification required.
• Ability to push, pull, and/or lift up to 50 pounds.

Responsibilities

• Provide coordination and support for patients with heart failure.
• Coordinate individualized, evidence-based care plans.
• Actively participate in process and quality improvement initiatives.
• Ensure accurate and timely documentation in EPIC.
