Job summary

Qualifications

• Current Oregon and CA RN License; BLS and ACLS certification by end of month from 60 days of hire.
• Willing to work varied hours with flexible schedules.
• Excellent communication skills both orally and written.
• Preferred BSN; Minimum 2 years’ experience in Cardiology, ICU or Heart Failure.
• Current Oregon and California RN license, willingness to obtain CHFN certification.

Responsibilities

• Serve as primary contact for heart failure patients, navigating care and treatment plans.
• Provide education on heart failure self-management, including meds and lifestyle changes.
• Conduct assessments, monitor progress, and adjust care plans as needed.
• Document interactions in the EHR and track patient outcomes for quality improvement.
• Coordinate between patients, families, providers, and the care team; support telehealth follow-up.
