Job summary

MercyOne Des Moines Medical Center seeks an RN Navigator: Heart Failure to coordinate and provide care for high-risk populations in inpatient and community settings. You will work with a multidisciplinary team to identify health care needs and educate patients and families to ensure quality, cost-effective care.

The role requires Iowa RN licensure and a BSN is preferred; 4 years of specialty experience and BLS certification are expected.

Qualifications

• Current Iowa RN license required.
• Bachelor’s degree in health-related field required or actively pursuing; BSN preferred.
• Demonstrates competency with 4 years experience in that specialty.
• Proof of completion of Mandatory Reporter – Child and Dependent Adult Abuse training within 3 months of hire.
• BLS certification within 6 weeks of hire (AHA BLS for Healthcare Providers).
• Demonstrates success in positions of leadership.

Responsibilities

• Coordinate care for groups of high-risk individuals in inpatient/community settings.
• Interact with patients, families and healthcare team to ensure education and plan of care.
• Develop individualized care plans across the continuum of care.
• Provide clinical leadership and precept visiting professionals, students and new graduates.
• Collaborate on education materials for patients and families to improve outcomes.

Education

Current Iowa registered nurse (RN) license

Bachelor’s degree in health-related field required or actively pursuing. BSN preferred.

4 years experience in that specialty

Mandatory Reporter – Child and Dependent Adult Abuse training

Basic Life Support certification (BLS) for Healthcare Providers
