Job summary

MercyOne Iowa Heart Center Des Moines is hiring an RN Navigator: Heart Failure/Cardiac to join our team at the Des Moines campus. You will develop and implement a Center of Excellence for Heart Failure Management using evidence-based guidelines and coordinate care transitions for patients and families.

The role emphasizes patient education, advocacy, and collaboration with physicians, nurses, and extended care services to ensure seamless care across inpatient, outpatient, and post-acute

Qualifications

• Registered nurse with Iowa license.
• BSN preferred.
• 2+ years of related work experience; 4 years preferred.
• Mandatory Reporter training within 3 months of hire.
• Excellent communication and presentation skills; patient advocacy.

Responsibilities

• Coordinate care across the diagnosis through discharge planning.
• Provide patient/family education and materials as directed.
• Collaborate with physicians and the heart failure team to ensure continuity of care.
• Follow-up post discharge as required.
• Reconcile daily census and conduct rounds for heart failure patients.
