TuftsMedicine is seeking a Heart Failure Nurse Navigator at Onsite MelroseWakefield Hospital to guide CHF patients through hospitalization, discharge, and recovery. You will coordinate care with cardiology teams, case management, pharmacy, and primary care to optimize outcomes and reduce readmissions.

You will identify high-risk patients, provide education on disease management, and ensure timely follow-up appointments while supporting caregivers and families in navigating the healthcare system.

Competenze

• Massachusetts RN licensure required.
• 3 years of acute care RN experience.
• Current Basic Life Support (BLS) certification.

Mansioni

• Coordinates multidisciplinary care across the care continuum.
• Provides communication, advocacy, and education in a culturally competent manner.
• Triages new patient referrals to ensure timely access to care.
• Conducts post-discharge follow-up calls and acts as liaison for navigating the healthcare system.
• Educates patients and uses adult learning principles to support self-management.
• Collects data on barriers, hospital experiences, and treatment plans.
• Collaborates with the care team to expedite patient care sequences.
