Job summary

CardioOne, in Londonderry, NH, seeks a Heart Failure Program Care Coordinator to support ongoing patient care in the Heart Failure Program. You will act as the central liaison among patients, families, cardiologists, APPs, and staff to ensure timely follow-ups and coordinated care.

The role focuses on improving outcomes, reducing readmissions, and ensuring continuity of care across the continuum, reporting to the Clinical Nurse Manager. Strong communication and care coordination are essential.

Qualifications

• Certified Medical Assistant or Licensed Practical Nurse preferred.
• Experience with heart failure patients is strongly preferred.
• Proficient in patient education and care coordination.

Responsibilities

• Coordinate care for Heart Failure Program patients from discharge through outpatient management.
• Review discharge summaries, notes, tests, and medication changes to update care plans.
• Schedule timely post-discharge follow-ups with cardiologists and APPs.
• Coordinate diagnostic testing including echocardiograms and labs.
• Monitor patient symptoms and vital signs; escalate clinical concerns as needed.
• Provide patient education on self-management, medications, and adherence.
• Document all patient interactions and track program enrollment and outcomes.
