Job summary

Tufts Medicine Integrated Network is seeking a Heart Failure Nurse Navigator to serve as a clinical expert, patient advocate, and care coordinator for CHF patients across the care continuum at MelroseWakefield Hospital. Qualifications

• Massachusetts RN licensure required.
• Minimum 3 years of acute care RN experience.
• Current BLS certification required.
• BSN preferred.
• Certification in Heart Failure Nursing (CHFN), Cardiac-Vascular Nursing (CV-BC), Case Management (CCM) or related preferred.

Responsibilities

• Assess and coordinate multidisciplinary care across the continuum.
• Provide communication, advocacy, and education in a culturally competent manner.
• Triage new patient referrals for timely access to care.
• Conduct post-discharge follow-up calls and act as liaison for patient navigation.
• Educate patients and families using adult learning principles for complete, connected care.
• Collect and report data on barriers, hospital experience, medications, prior treatments.
• Collaborate with care team to expedite patient care sequence.
• Serve as liaison between patient and providers to address needs.
• Ensure patient needs are integrated into the care plan.
• Encourage patient participation in decision making about treatment and care.
