Job summary

University Health seeks a CHF Clinical Coordinator to oversee coordination of care for heart failure patients in an outpatient setting. You will guide patient care staff, deliver direct nursing services, and triage patient questions following physician protocols.

Responsibilities include staff orientation, interprofessional collaboration, and patient education as directed by the physician, ensuring quality and timely care within a multidisciplinary CHF program.

Qualifications

• RN licensure in Texas (required).
• BSN preferred but not mandatory.
• Minimum 2 years of recent clinical experience in cardiology.
• Familiarity with heart failure care and patient education is expected.

Responsibilities

• Coordinate services for heart failure patients with a multidisciplinary team.
• Provide direct patient care and clinical support as needed.
• Perform phone triage of patient questions using physician protocols.
• Orient and train clinical support staff; serve as a resource on CHF care.
